Goal # 1

Assure Timely Access to Treatment Services

Narrative

Many individuals seek treatment during a time of crisis or exacerbated symptomatology.     It is critical to make timely accommodations to initiate treatment services.  A delay in initiating treatment could deter a client from entering treatment or could result in client’s loss of faith in the organization’s ability to meet his or her needs.

2018 Goal 

Ninety percent (90%) of intake referrals will be placed within 6 weeks.  The average length of time between initial inquiry and recovery housing admission will not exceed 60 days.

Result

In 2018, Cedar Ridge Recovery Housing admitted 27 total residents.  Of those, 22 (81.5%) were able to be placed within 6 weeks of initial inquiry.     The average length of time between inquiry and recovery housing admission was 25.3 days.     

Goal # 2

Enhance the Rate of Clients Discharged Due to Meeting Treatment Goals

Narrative

Clients discharge from treatment for various reasons, both positive and negative.  A successful discharge is one in which all treatment goals have been met, and client no longer requires services through the organization.  A successful discharge following the client’s treatment experience is often reflective of high-quality services provided by the organization.     

2018 Goal 

Forty-five percent (45%) of all discharges from recovery housing will be successful as evidenced by favorable completion of the goals outlined in the client’s treatment plan.

Result

4th Quarter 2018:  66.6% of clients discharged due-to meeting treatment goals. 

Goal # 3

Enhance Overall Client Satisfaction

Narrative

A client’s level of satisfaction is crucial as it provides a first-hand account of the client’s impressions regarding the organization, treatment providers, facilities and their overall treatment experience.  Clients who are satisfied with their treatment experience are more likely to recommend the organization to friends, family members and other acquaintances within the community - thereby enhancing the organizations reputation and vitality.    

2018 Goal 

Ninety percent (90%) overall satisfaction rate will be reported via anonymous satisfaction surveys collected from clients served.  

Result

A total of 44 satisfaction surveys were submitted by Cedar Ridge’s recovery housing residents.  The overall satisfaction rate for the year was 94.8%.  

Goal # 4

Enhance Overall Referral Source Satisfaction

Narrative

Referral source feedback can provide a first-hand account of the of the experience of coordinating and linking clients with the services provided by the organization.   Referral sources who are satisfied with their experience are more likely to refer client’s again - thereby enhancing the organizations reputation and vitality.      

2018 Goal 

Ninety percent (90%) overall satisfaction rate will be reported via anonymous satisfaction surveys collected from referral sources. 

Result

In 2018, 28 referral source satisfaction surveys were returned to the organization. 

The average referral source satisfaction rate for 2018 = 87.00%
