Goal # 1

Assure Timely Access to Treatment Services

Narrative

Many individuals seek treatment during a time of crisis or exacerbated symptomatology. It is critical to make timely accommodations to initiate treatment services. A delay in initiating treatment could deter a client from entering treatment or could result in client’s loss of faith in the organization’s ability to meet his or her needs. 

2021 Goal 

Ninety percent (90%) of intake assessments will be scheduled within 14 days of request for services from referral source .

Result

Cambridge Outpatient had an average of 87% of intakes were completed within 14 days of request. Zanesville Outpatient had an average of 81% of intakes were completed within 14 days of request. 

Goal # 2

Improve Client Attendance at Scheduled Treatment Appointments 

Narrative

A client’s failure to appear for a scheduled treatment appointment is not only an inefficient use of staff time, but an indicator that the client is not receiving services at the frequency prescribed by the treatment plan, which may indirectly delay (or otherwise negatively impact) treatment outcomes. 

2021 Goal 

Seventy percent (70%) of all scheduled treatment appointments will be attended 

Result

Cambridge Outpatient had a 73 percent (73%) show (attended rate) for all scheduled appointments. Zanesville Outpatient had a 74 percent (74%) show (attended rate) for all scheduled appointments.     

Goal # 3

Enhance Overall Referral Source Satisfaction

Narrative

Referral source feedback can provide a first-hand account of the experience of coordinating and linking clients with the services provided by the organization. Referral sources who are satisfied with their experience are more likely to refer clients again - thereby enhancing the organization’s reputation and vitality.      

2021 Goal 

Ninety percent (90%) overall satisfaction rate will be reported via anonymous satisfaction surveys collected from referral sources.

Overall Stakeholder Satisfaction (%) for 2021 was 88%
